We are looking for a competent substitute Caregiver to help provide coverage on various shifts.

Caring for our terminally ill guests in a professional and compassionate manner is essential.

Healthcare experience preferred but not required.

All training is provided.

Must be over the age of 18 by NYS law.

*Job Summary: *

Surrogate Caregivers provide direct personal care to Abraham House's terminally ill guests consistent with Abraham House's policies and procedures.

*Essential Responsibilities and Duties for Surrogate Caregivers: *
* Participates in orientation, training and ongoing professional development as needed to ensure best standards of practice for care giving and ensures understanding and adherence to Abraham House's procedures.
* Ensures needed care is provided for our guest's ultimate comfort, dignity and care.
* Arrives timely for shifts and notifies in a timely manner whenever unable to keep a scheduled shift.
* Signs in and out on Homebase app.
* Administers medications according to the Medication Administration Policy & Procedure.
* Consults with the Assistant Director / Executive Director as needed in regard to daily operations and guest services
* Serves as liaison between Hospice & Palliative Care, Inc. (HPCI) and guests as appropriate. Contacts HPCI as appropriate when there are changes in guest's symptoms or questions regarding care. Adheres to Abraham House's procedure on "Notification of Hospice & Palliative Care, Inc." and "When to Call Hospice" guidelines.
* Documents appropriately and timely in guest journals and in the Communication Book.
* Ensures light meals/snacks are provided as appropriate to the guests.
* Provides direct care relating to guest health and hygiene.
* Coordinates to ensure needed supplies are stocked and available for guests' comfort, dignity and care.
* Ensures that universal precautions are utilized and rooms are appropriately disinfected per policy upon guest's death.
* Completes Discharge Checklist as appropriate upon guest's death or discharge to another setting.
* Establishes positive working relationships with paid and volunteer surrogate caregivers, administration and the public.
* Provides appropriate emotional support to guests and families.
* Interacts with community based individuals in a sensitive and professional manner.
* Performs additional related duties and responsibilities as required.

*Education, Training and Experience for Surrogate Caregivers: *

Since Abraham House is a Comfort House which is based on a model of a family caring for a loved one at home, there is no license that is required to be a surrogate caregiver. Previous healthcare experience is helpful but not required. A surrogate caregiver must demonstrate to the satisfaction of the Assistant Director, Executive Director and Trainers the ability to provide care for guests.

*Additional Qualifications, Special Skills, & Other Requirements for Surrogate Caregivers: *
* Ability to work effectively with terminally ill guests and their families.
* Manage multiple tasks, noise, interruptions, stress, and emotional loss.
* Work well independently, cooperatively and effectively with volunteers and other staff while maintaining a professional demeanor.
* Possess strong time management skills
* Maintain a commitment to the Abraham House mission.
* Maintain high standards of confidentiality including all HIPAA standards.
* Must have excellent communication, listening, documentation and telephone skills.
* Must have an understanding of a variety of cultural, ethnic and socio-economic backgrounds and an ability to execute a supportive and sensitive approach to guest's and family needs.
* Health status is consistent with the ability to meet job requirements. There must be no physical or mental handicap which would interfere with the proper performance of duties.

*Occupational exposures, hazards, work environment: *

All surrogate caregiver paid time will be within Abraham House. Has exposure to terminally ill guests, periods of stress, variable activities, and variable acuity of work load, flexible hours, lifting, occasional step climbing and use of telephone.

*Supervision Received and Exercised for Surrogate Caregivers: *
* Receives general supervision from Assistant Director, Executive Director and Caregiver Trainers.

Job Type: Part-time

Pay: $17.50 per hour
