Job Description:
Main Function: Provide person centered services and supports to
assist the individuals during Community-Based services to achieve
independence and maintain their health and safety. Responsibilities
include: promoting growth and independence with personal needs and
ADLs, monitoring health and safety, encouraging self-advocacy and
development of natural supports, situational counseling, benefit
and entitlement counseling, and completion of required
documentation.Duties and Responsibilities of a Community Support
Specialist Relief:Provision of services for participantsImplement
participants' goals as written in Service PlanSupport participants
by providing assistance with daily living skills (ADLs) and
oversight as neededPromote advocacy with participants by seeking
out services, promoting choice, and assisting in their
understanding of rights and responsibilitiesEncourage and support
opportunities that promote natural connections within each
participant's communityAssist with the identification of
participants' areas of need and assist with coordination of
services and delegate supports appropriatelyAssist participants
with identifying need and barriers in the areas of housing,
socialization, education, employment, and physical/mental health,
in order to promote overall well-being Develop and implement
programs/activities that address the physical and mental health
needs of participants. Provide support in individual as well as
group settingsRespond to crisis situations by using good judgment
through prevention, intervention, resolution, and reporting to
ensure the safety and well-being of participantsAssist in the
referral processes for agency and community programs in a timely
manner Completes required documentationComplete all program
documentation in a timely manner and per Agency standards.
Documentation includes but not limited to: service plans, service
notes, financial, and program outcomes, etc.Collaborates with
participants to write service plans in a person-centered way,
focusing primarily on physical and mental healthCompletes incident
report forms thoroughly and in a timely manner per Agency
standardsCompletes mileage sheets per Agency standards Community
and TeamworkWork collaboratively with participants, families,
program staff, co-workers and supervisorPossess knowledge/awareness
of a broad range of community resources that promote mental and
physical health and encourages participant involvement in areas of
interestAttend staff meetings and other meetings/committees as
directed by supervisorAttend all required trainings as directed by
supervisor and maintain training certificationsParticipate in
Agency-wide events, advocacy, and/or community education. Follows
Agency standards and proceduresProvides person-centered, quality
servicesRespect participant privacy and follow confidentiality and
non-disclosure laws and policiesFollows Agency standards and
proceduresMaintains compliance with applicable rules and
regulationsMeets attendance and punctuality standards Other duties
as assignedEducation/Experience:High school diploma with 1 year
experience working with people with a mental health diagnosis
required. Associate's degree in a Human Service field preferred.
Basic word processing and e-mail skills are required. Conditions of
Employment: Must have a valid New York State driver's license in
accordance with the Agency Vehicle and Driving policy and
transportation necessary to meet program needs. Must be able to
lift 75 pounds. Successful completion of required orientation and
annual training. Must be able to demonstrate teamwork and
professionalism. Must be able and willing to demonstrate direct
care duties, including personal assistance, as need arises. Must
participate and meet established requirements in a database check
through the Division of Criminal Justice Services as defined by the
State of New York, Office of Mental Health, Chapter 575 of the Laws
of 2004. Must participate in successful database checks through the
HHS Office of Inspector General, Excluded Parties List System, New
York Office of Medicaid Inspector General and Specially Designated
