Job Description:

The RN Care Management Team Leader provides clinical support to care management staff, including supervisors, care managers and education department. This position is semi-remote (work from home with periodic patient visits and monthly team meetings at the company's Syracuse, NY office). Mileage is compensated from your home at the IRS reimbursement rate. This opportunity is perfect for the Registered Nurse living in the Syracuse, Baldwinsville, Skaneateles, Tully, Fayetteville, Manlius, or Cicero areas, who has previous Managed Long Term Care, home care, or case management experience and is seeking a regular schedule with no nights, no weekends, no holidays required.Performance Responsibilities and Standards:* Serves as an expert consultant to Care Managers.* Serves as a lead in the orientation of new personnel, mentors peers and promotes collaborative teamwork* Through joint visits, will mentor/precept staff to the role of Care Management, ensuring that staff meets the job position expectations safely and timely.* Completes supervisory visits with care management staff to ensure competency in skill set such as assessment of member services, coordination of member care, and coordination of community agencies per regulatory/agency guidelines under direction of Care Management Supervisor.* Completes UAS-NY validation visits with care managers per DOH regulations as needed* Assists with employee reviews and performance appraisals of the care management staff under direction of Care Management Supervisor.* Provides back-up support to supervisor covering for Team Supervisor during times of absence.* Completes care management visits to members per agency need.* Oversees pre-enrollment applicants that are potential transfers back to community setting that have been assessed for MLTC needs and awaiting finalized discharge plans and determination of care plan. Collaborates with appropriate facility discharge planner when members transition between settings.* Works collaboratively with Education Department in providing education to Care Management team. Also verifies attendance and ensures that education is provided to those that are absent or need reinforcement.* Participates in regular team meetings and activities. * Participates in departmental and organizational committees and meetings as applicable.* Maintains a working knowledge of, and adheres to applicable federal/state regulations including, but not limited to patient confidentiality, release of information, and HIPAA.* Interacts in a manner, which is professional, respectful, positive, helpful, and promotes trust and commitment to co-worker and maintains appropriate professional boundaries.* Attends agency in-services and educational programs and maintains professional growth and development.* Participates in the quality improvement and utilization review activities of the organization.* Clarifies issues and actively addresses concerns in a timely manner.* Performs other related duties as requested.Requirements:* Strong organizational and time management skills.* Ability to manage conflict, stress and multiple simultaneous work demands in an effective, professional manner.* Ability and willingness to self-motivate, prioritize, and be willing to change processes to improve effectiveness efficiency. Decisions and problem solving require a combination of analysis, evaluation, and interpretive thinking.* Consistently meets position agreed-to goals and objectives.* Computer literacy, including but not limited to data entry, retrieval, and report generation.* Ability to work with patients/families of all ages and in a variety of settings, including inpatient facility and patients' homes presenting diverse physical conditions and social/cultural environments.* Excellent written, verbal and listening communication skills.* Listens to or acknowledges the viewpoints of others.* Sends clear, consistent messages to members/family/staff.* Utilizes varied communication styles to communicate to produce results.* An understanding of Medicare/Medicaid regulatory and entitlements.* Knowledge of chronic disease management.* Demonstrated ability to work with a multi-discipline team.Qualifications:* Licensed and currently registered to practice as a Registered Professional Nurse in New York State, with minimum of 1-year medical surgical experience.
