Patient Care Coordinator, LPN preferred but not required Administration: Support Nurse: Coordinator Syracuse, NY 13201 Full-time licenses and certifications NY License, RN minimum education High School/GED years experience 3+ years Job Title Patient Care Coordinator Job Description The Patient Care Coordinator provides support to clinical field staff, the House Resource Manager, Clinical Supervisors and the Vice President of Patient Services through completion of scheduling, clerical tasks and coordination. Required Skills and Experience - High School Diploma or G.E.D., LPN preferred but not required, with a minimum 3-5 -year medical office/ customer service experience. - Computer proficiency with Microsoft office, or other word processing, spreadsheet, and database management. - Excellent organizational skills with the ability to prioritize multiple tasks. - Excellent customer service both to internal and external customers. - Ability to communicate effectively in English, both verbally and in writing.
