Physical Therapist If you are a Physical Therapist looking to make

provides skilled nursing and therapy services in patients homes
throughout Jefferson County, NY. This comfortable alternative to
inpatient care allows people to recover from an accident or illness
in a familiar environment surrounded by family and friends. Shift:
Days (40 hours) Monday- Friday, no weekends or major holidays.
Responsibilities: Performs initial evaluations and re-evaluation.
Plans and implements a physical therapy program for medically
referred patients, residents, or registrants. Physical Therapist
may assist with supervising support staff. Requirements: Graduation
from an American Physical Therapy Association accredited Physical
Therapist education program. Successful completion of all Physical
Therapist student internships New York State registration. New York
State Physical Therapy License required. BLS certification
required, or obtained within 3 months of hire date. Benefits: 401k
