Foltsbrook Care Center in Herkimer, NY is seeking an experienced
LPNAC to join our team on a full-time basis. In this role, you’ll
support our nursing facility by completing MDS assessments and
ensuring accurate documentation for quality resident care and
proper reimbursement. Key Responsibilities Complete MDS assessments
(MDS experience required) Prepare and transmit reports to Medicare
& Medicaid Ensure accuracy of medical records and coding Conduct
care area assessments and care planning Skills, Knowledge and
Expertise Licensed Practical Nurse (LPN) in the State of NY with
MDS experience Strong attention to detail and accuracy Ability to
