Worker (CHW) Access, Case Management, & Community Outreach (CHW I,
CHW II, CHW III – Level determined based on experience and
competencies) Position Summary: The Community Health Worker (CHW)
is a trusted member of the community who supports patients in
accessing healthcare services, navigating systems of care, and
addressing social and cultural barriers to health. The CHW serves
as a liaison between patients, healthcare providers, and community
resources, providing culturally and linguistically responsive
support to improve health outcomes. This position may be filled at
the CHW I, CHW II, or CHW III level, depending on experience,
demonstrated competencies, and program needs. Key Responsibilities:
(Responsibilities may vary based on assigned CHW level) Maintain
strict confidentiality and comply with HIPAA guidelines and
organizational policies Assist patients with navigating the
healthcare system and accessing health-related services Register
patients and schedule, confirm, and coordinate medical appointments
Conduct informal or structured needs assessments (AHC-HRSN), as
appropriate Identify and address barriers to care such as
transportation, language, literacy, and financial hardship
Coordinate transportation and other access-related services Provide
Spanish interpretation and written translation services in
accordance with training and scope Link patients to community
resources for food, housing, financial assistance, insurance, and
medication support Provide education on health promotion and
disease prevention using approved materials Support outreach and
community engagement activities Document all patient interactions
and services in the electronic health record (EHR) Collaborate with
clinical, care management, and outreach teams to ensure coordinated
care Some CHW roles will have more emphasis on Community Outreach
and Engagement while others will be more focused on site, depending
on clinic need. Requirements: CHW Levels & Scope: CHW I – Access &
Navigation: Entry-level role focused on phone coverage, scheduling,
intake, basic interpretation, transportation coordination, and
referrals without ongoing case ownership CHW II – Case Management &
Outreach: Mid-level role providing case management, follow-up,
benefits navigation, outreach, and advanced interpretation CHW III
– Lead / Specialist CHW: Advanced, non-supervisory role managing
complex cases, providing training and mentoring, supporting
workflow improvement, and serving as a subject-matter expert
Qualifications: High school diploma or equivalent required CHW
certification required or preferred based on level and state
requirements Bilingual (Spanish/English) strongly preferred
Experience in community health, social services, outreach, or
healthcare preferred Strong interpersonal, communication, and
organizational skills Ability to work with diverse populations and
maintain professional boundaries Basic to advanced computer skills,
including EHR documentation Level Placement: CHW level (I, II, or
III) will be determined based on: Relevant experience and training
Demonstrated competencies and skills Program needs and scope of
responsibilities
