Paradigm, a leader in catastrophic and complex case management, is seeking a dedicated and detail-oriented Catastrophic / Field Nurse Case Manager to join our team. This role involves working directly with patients who have suffered catastrophic injuries, ensuring they receive the best possible care tailored to their unique needs. Our goal is to optimize health outcomes while managing costs, through personalized and compassionate case management services. Conduct thorough assessments of new cases, determining patient needs and potential care plans. Collaborate with healthcare providers, insurers, and patients to develop and coordinate effective care plans. Monitor and report on patient status and progress, adjusting care plans as necessary. Maintain accurate and up-to-date patient records in accordance with legal and ethical standards. Provide support, education, and resources to patients and their families to facilitate understanding and decision-making. Advocate for patients’ needs and rights for accessing necessary healthcare services. Travel to patient locations as required to directly assess and evaluate health status and environments. Strong clinical skills with an ability to handle complex medical information Excellent communication and interpersonal skills High organizational and problem-solving abilities Capability to work independently in the field Proficient in medical software and Microsoft Office Suite Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid Registered Nurse (RN) license. Certification in Case Management (CCM) preferred.
