Care (SCC) program is a longitudinal, integrated care delivery
program that coordinates the delivery and provision of clinical
care of members in a facility setting. The APC within SCC provides
care to our highest-risk health plan members and is part of an
interdisciplinary team. In this role you will have the ability to
achieve work life balance. No on-call, no weekends, and no holidays
required. This position requires travel through the state of New
York. /n /n Primary Responsibilities: /n /n /n - Provide clinical
coverage for the primary APC during periods of PTO or short/long
term absences /n - Manage members through an acute medical
situation or an exacerbation of a chronic condition, including
testing, diagnostics, and interventions /n - Manage members during
a short term, sub-acute rehabilitation episode /n - Conduct
comprehensive assessments /n - Effectively manage acute and chronic
medical and behavioral conditions /n - Ensure accurate and complete
documentation, including ICD 10 conditions /n - Communicate and
collaborate with the interdisciplinary care team /n - Conduct
advanced illness and advanced care planning conversations /n -
Provide patients and caregivers with counseling and education /n /n

Qualifications: /n /n - Active and unrestricted license in the
state of Florida or ability to obtain by start date /n - Certified
Nurse Practitioner or Physician Assistant through a national
board/n /n - For NPs: Graduate of an accredited master’s degree in
Nursing (MSN) program and board certified through the American
Academy of Nurse Practitioners (AANP) or the American Nurses
Credentialing Center (ANCC), Adult-Gerontology Acute Care Nurse
Practitioners (AG AC NP), Adult/Family or Gerontology Nurse
Practitioners (ACNP), with preferred certification as ANP, FNP, or
GNP /n - For PAs: Graduate of an accredited Physician Assistant
degree program and currently board certified by the National
Commission on Certification of Physician Assistants (NCCPA) /n /n
/n - Current, active DEA licensure/prescriptive authority or
ability to obtain post-hire, per state regulations (unless
prohibited in state of practice) /n - 1 years of hands-on post grad
experience (long-term care setting preferred) /n - Ability to gain
a collaborative practice agreement, if applicable in your state /n
- Ability to complete physical requirements of the job including
lifting a 30-pound bag in and out of the car, navigating stairs and
a variety of dwelling conditions, and sit, stand, and kneel to
perform physical assessment /n - Driver's License and access to
reliable transportation /n /n /n Preferred Qualifications: /n /n -
Experience in meeting the medical needs of patients with complex
behavioral, social, and/or functional skills /n - Understanding of
Geriatrics and Chronic Illness /n - Understanding of Advanced
Illness and end of life discussions /n - Proficient computer skills
including the ability to document medical information with written
and electronic medical records /n /n /n The salary range for this
role is $89,800 to $176,700 annually based on full-time employment.
Pay is based on several factors including but not limited to local
labor markets, education, work experience, certifications, etc.

NOTE Employees must be in an active regular status. If an employee
leaves Home and Community, the student loan repayments will cease.
The employee must remain in an Advanced Practice Clinician or
Physician role within Home and Community for 36 months to receive
