Job Description Job Description Job Title: Lead Primary Therapist Department: Outpatient behavioral health Location: 35 North St 300, Canandaigua, NY 14424 Hours Per Week: 40 Schedule: Monday - Friday; Days, 8:00AM - 4:30PM SUMMARY: The Primary Therapist provides mental health diagnostic assessment, treatment planning, and mental health therapies for persons suffering from mental illness. Treatment is provided in the context of a clinical team, supervised by the staff psychiatrist. Leadership and/or supervisory duties as delegated by manager. Provide superior customer service by modeling the Brand Promise and Core Values. RESPONSIBILITIES: - Provides initial and ongoing assessment, counseling and clinical intervention to a designated caseload. - Provides individual, group, and family psychotherapy. - Participates functionally as a member of an interdisciplinary team in coordination and delivery of patient care. - Communicates in a timely and complete manner with internal and external care providers to complete ongoing assessment and to coordinate care. - Participates in and incorporates professional supervision and ongoing education toward quality care. - Utilizes efficient computer skills for electronic documentation. - Completes all required documentation in a timely and comprehensive manner. - Maintains a caseload, meeting productivity requirements as stated by the Department of Psychiatry and Behavioral Health, and seeks to keep the incidence of failed appointments to a minimum. - Must maintain licensure in a timely manner. - Undertakes planning, problem solving and quality improvement activities regarding program functioning. - Pursues personal and professional growth using internal and external sources available for supervision/consultation to staff and provides direction, feedback, and positive feedback to supervisors. - Provides clinical supervision/consultation to the clinical team or program staff as delegated by manager. - Assists Team Leader/Manager in assuring smooth, effective delivery of care in response to community requests for services, as delegated. REQUIRED QUALIFICATIONS: - LCSW, LMHC, LMSW, LMFT, or LCAT - Master’s Degree in Human Services field required. PREFERRED QUALIFICATIONS: - Five to seven year of experience preferred. EDUCATION: MS (Required) LICENSES / CERTIFICATIONS: PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met. PAY RANGE: $69,500.00 - $94,500.00 CITY: Canandaigua POSTAL CODE: 14424 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
