Pay Information $2,044 per week About The Position Job Title: Pediatric RN - Home Care Profession: Registered Nurse Specialty: Case Manager RN Duration: 13 weeks Shift: Weekends Hours per Shift: 8 hours Experience: One year of experience as a clinical pediatric nurse strongly preferred License: New York State Licensed RN Certifications: BLS, ACLS Must-Have: Certified Home Health Care Experience Description: Develop care plans appropriate to the patients' and families' healthcare needs in conjunction with the patient, family, physician, clinical operations team, and third party payers. Obtain medical orders and approval of medical services. Complete the OASIS Assessment as needed to accurately reflect the clinical, functional, and social needs of the patient. Ensure the completion of all appropriate clinical records needed for compliance with state and federal legislation and seamless service delivery within established time frames. Refer to appropriate health care systems and community agencies/resources. Communicate the outcomes of the assessment/reassessment visits to the healthcare team members to ensure appropriate follow-up occurs based on the patient's and family's plan of care. Display an exemplary level of patience, courtesy, and flexibility. Interact with patients, physicians, and staff in a manner conducive to maintaining positive relationships and meeting the goals and objectives of the agency. Ensure problem resolution occurs at the point of contact. Maintain appropriate productivity requirements. Adhere to professional home visit performance standards. Possess strong interviewing, assessment, and communication skills. Exercise initiative and independent judgment. Build and maintain professional relationships. Work independently with minimal supervision and adapt to change. Manage a variety of assignments and complete duties as assigned.
