TeamworkSupport individuals served by providing assistance with daily living skills and oversight as neededMaintain ongoing relationship with community residence personnel as assigned to facilitate discharge planning.Possess a broad knowledge of community services which can be made available to participants, family members, services providers, and other significant support persons. Communicate effectively with relevant Agency residential, outreach, and clerical staff to maximize support available to program participants.Maintain appropriate service, statistical, and financial records as required for coordination of services with other team members in a manner which respects confidentialityCoordinate services with such programs to identify participants' areas of need and delegate supports appropriatelyInitiate and maintain ongoing communication with participants and key people integral to achieving goals and providing servicesDemonstrate professional accountability through respect, open communication, and active participation in a team environment.4. AccountabilityAssist in the referral processes for agency and community programs and ensure responses to referrals are conducted in a timely manner. Coordinate services with such programs to identify participants' areas of need and delegate supports appropriately.Complete documentation based on program requirements following established guidelines and timeframesRespond to crisis situations by using good judgment through prevention, intervention, resolution, and reporting to ensure the safety and wellbeing of the individual5. InnovationPromote advocacy with individuals by seeking out services, promoting choice, and assisting in their understanding of rights and responsibilitiesTake an active advocacy stance for participants while teaching them to advocate for themselves whenever possible to facilitate skill development 6. Program FunctionsProvide person centered educational rehabilitation services to program participants which includes but is not limited to:-- Assessment of student strengths and possible barriers to educational success-- Assist students with developing educational plans-- Provide career counseling as it relates to students' educational goals-- Provide guidance relative to the admissions, registration, and financial aid processes, including rehabilitating/discharging deferring student loans.-- Assist students with developing the skills and supports necessary for success in the educational environment of their choice-- Provide limited academic support (e.g. editing papers, test preparation)-- Advocate for students as needed and promote self-advocacy skills with students-- Make referrals to other resources and assist with accessing resources as needed-- Develop and facilitate group instruction as needed-- Facilitate social connections/events that enhance the students integration into academic lifeScope of Responsibility:In addition to program-specific functions, responsibilities will include but not be limited to: assisting participants with establishing and maintaining both mental and physical health care, teaching and reinforcing a wide range of daily living skills, promoting independence in medication self-administration, providing situational counseling, benefits and entitlement counseling, engaging in crisis intervention, encouraging self-advocacy and facilitating the development of natural supports. All services will be provided in collaboration with program participants, utilizing a person-centered approach. Supports will be provided at the degree to which participant's desire. Education and Experience:Bachelor's Degree preferred, Associate's Degree plus three years of experience in human services or higher education accepted, or equivalent combination of relevant education and experience. Experience in the mental health field preferred. Conditions of Employment:Must have a valid NYS driver license in accordance with the Agency Vehicle and Driving Policy and transportation necessary to meet program needs. Schedule flexibility to accommodate participant needs, which will include evenings and weekends. Possess good verbal and listening communication skills. Must be physically capable of performing job functions and must work well independently. May need to be outside in extremely cold, hot, or wet weather. Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the New York State Office of Mental Health. Must participate in successful database checks through the HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specially Designated Nationals.
